However, experts confirm that “clinical-grade” is essentially a meaningless buzzword, crafted to imply rigor and trustworthiness without the burden of actual medical oversight or accountability.

This type of language belongs to a broader category of marketing puffery that borrows credibility from science and medicine. Similar terms include “medical-grade” for materials like steel or supplements, “prescription-strength” for skincare products, and “hypoallergenic” for cosmetics, all of which sound impressive but lack standardized definitions or testing protocols. Lyra’s executives have openly stated they do not believe their product falls under FDA regulation, admitting that the clinical language serves mainly to differentiate their offering in a competitive marketplace.

Lyra presents its AI as a supplementary tool within an existing framework of human-provided mental healthcare, offering users continuous access to support between therapy sessions. The chatbot reportedly draws from past clinical conversations, offers resources such as relaxation exercises, and applies unspecified therapeutic methods. Still, the company has not clarified what exactly makes its AI “clinical-grade,” and it did not respond to requests for a definition.

According to George Horvath, a physician and law professor at UC Law San Francisco, there is no regulatory meaning to the term “clinical-grade AI.” He notes that the FDA has never referenced the phrase in any official document, statute, or regulation. Instead, it appears to be an industry-coined expression that allows each company to assign its own interpretation.

Vaile Wright, a licensed psychologist and senior director at the American Psychological Association, observes that companies adopt such ambiguous language intentionally. By using terms that sound scientific but avoid regulatory scrutiny, they sidestep the expensive and lengthy FDA approval process, which requires rigorous clinical trials to prove safety and effectiveness. Wright points out that this kind of “fuzzy language” is legally permissible, even if it confuses consumers, because current regulatory pathways were not designed with rapidly evolving digital technologies in mind.

Beyond the FDA, the Federal Trade Commission holds authority to intervene when marketing becomes deceptive. FTC Chairman Andrew Ferguson has already initiated an inquiry into AI chatbots, particularly concerning their impact on minors, though the agency has not yet commented on terms like “clinical-grade.”

Stephen Gilbert, a professor of medical device regulatory science in Germany, believes that if companies can legally, or even illegally, get away with making vague claims, they will continue to do so. He suggests that regulators need to clarify and simplify requirements to close these loopholes.

This trend is not unique to artificial intelligence or mental health. The wellness industry is saturated with products boasting “clinically-tested” ingredients or “immune-boosting” benefits, all operating in a regulatory gray area. AI tools are simply adopting this established strategy of using impressive-sounding language that doesn’t hold up to close examination.

Companies often include careful disclaimers in their terms and conditions, explicitly stating that their products are not meant to replace professional medical care or diagnose illnesses. This legal wording helps them avoid classification as medical devices, even as users increasingly turn to these tools for therapeutic support without clinical supervision.

For example, Slingshot AI’s Ash app markets itself for “emotional health,” while Headspace promotes its AI companion Ebb as a “mind’s new best friend.” Both emphasize their role as wellness aids rather than medical tools. Even general-purpose chatbots like ChatGPT include disclaimers rejecting any formal medical use. The consistent message is one of functional ambiguity: behave like a therapeutic tool, but deny being one.

Regulators are beginning to take notice. The FDA had planned a meeting in November to discuss AI-enabled mental health devices, though it’s uncertain whether that will proceed. In the meantime, Lyra and similar companies may be treading a fine line. Horvath warns that if an AI tool begins diagnosing or treating conditions, it could easily cross into territory that qualifies it as a medical device.

Gilbert argues that it’s disingenuous for companies to use terms like “clinical-grade” while simultaneously denying they provide clinical services. In his view, the phrase is empty, a marketing tactic rather than a marker of quality or safety. Until clearer standards are established, consumers should approach such claims with healthy skepticism, recognizing that impressive language does not always reflect genuine clinical value.
