While the developers aimed to capture the essence of earlier Fallout titles through a focus on desolate environments, deadly deathclaws, and ruthless raider gangs, they avoided simply copying locations. Instead, they created a fresh landscape with its own identity, featuring subtle homages like an abandoned minigolf course dotted with familiar dinosaur statues. The heart of this new zone is a central hub known as Highway Town, which serves as the gateway to a brand-new bounty hunting system.

The bounty hunting mechanics form the core of the new gameplay loop and are directly tied to The Ghoul. This character, performing original dialogue written specifically for the game, assigns players various contracts to track down and eliminate targets across Ohio. Missions range from standard Grunt Hunts to more demanding Head Hunts. These larger encounters function as public events, allowing multiple players to join forces against powerful boss enemies. Initial hands-on experiences with the system involved straightforward objectives, such as locating and defeating a heavily armored enemy hiding among a group of raiders.

Beyond the bounty system, the update enriches the game with a fresh arsenal of weapons, new gear sets, and even additional types of fish to catch. Players can also acquire a new camp companion, a pig named Hog Meat. The narrative introduces new factions and characters, with early missions involving a plot to incite a rebellion against a tyrant known as the Rust King.
